Hi, I'm Dr. Jensen, a board certified general surgeon. I'm a member and fellow of the American Academy of Osteopathic Surgeons. I am also a member of the American Academy of Cosmetic Surgery. I've has been in practice and serving Davis and Weber counties for 20 years. My practice is dedicated to the surgical treatment of cosmetic concerns and varicose veins.
I've had my own personal journey with weight loss and as a result, I have placed a special emphasis on helping people with body contouring, especially, Tummy Tuck, Liposuction and Brachioplasty. Monique Berezay
Hello my name is Monique! I am an owner of Elevation Aesthetics alongside my husband Dr. Joseph Jensen. Dr. Jensen and I met through a dating website (like so many of us do now) and have been inseparable for the past 5 years and counting. We fell in love over sharing new thought- provoking ideas while hiking the many trails the Wasatch has to offer.
I bring with me over 20 years of experience in Business Administration. I have enjoyed each opportunity to build successful businesses. The excitement in hiring talented people that show passion for their work while delivering exceptional service is rewarding to me. I believe if you hire and provide training along with opportunities to grow professionally you create an environment where people will flourish. When your people love what they do it comes through without a doubt in how they treat our patients. My goal is to create an experience you are sure to love!
I wear many hats at the office, however my favorite hat to wear is mentor and coach. We all benefit having someone in our corner that listens to needs, concerns and goals. I am here to help create an experience for our team and patient that lead you finding your goals actualized.
In my free time, you'll find me spending time with my kids and Dr. Joe. I love being outside and exploring in nature. We love what we do, you can see and feel it in your results. We see you as one of a kind and look forward to seeing you!

Hi, I'm Heather, the Office Manager at Elevation Aesthetics. I’ve worked in a professional environment for over 20 years. I have a degree in Health Education and loved the 4+ years in which I worked in the medical field. I was a nutritionist and program manager for physicians who specialized in weight management. Being in a medical setting and offering high quality care to patients is truly an honor. I believe in helping others look and feel their very best. Outside of work, Heather loves trying new recipes, traveling with her family, and talking about her chickens.
Emily Grant- Office Assistant
Hi, I'm Emily! I'm the newest member of the amazing team here at Elevation Aesthetics! I love greeting clients and helping them receive the treatments they need. I bring with me over 20 years of cosmetic industry experience! I have been married to my loving husband Mike for just over 20 years and we have 4 awesome kids. I am so excited to start this journey with Elevation Aesthetics!

I'm Claire, Dr. Jensen's right-hand gal! I've worked in healthcare for over 25 yrs. I enjoy working with doctors who are compassionate and have a desire to help others. I have definitely found that here at Elevation. The team we have built is extraordinary. What I enjoy most is being a part of the life-changing surgeries performed at Elevation. Giving someone back the quality of life they so desire is such a beautiful and meaningful journey for the patient and myself. Outside of work, I love spending time with my daughters and grandchildren. I also love going to comedy shows and pretty much any outdoor activity. Jasmine Hamilton- Nurse Practitioner
Hi! I'm Jasmine. I am a registered nurse who is beyond excited to work in the aesthetics world. I specialize in dermal fillers and neuromodulators. I am a mom of 2 little girls who light up my life. Outside of work, I love spending time with my family, going to the pool, and eating a good donut. I can't wait to help men and women feel good about themselves, inside and out. Kohdi Hansen
My passion for this industry is so much more than skin! I have a love for connecting with patients and clients on a more intimate level. I love providing a boost of self-confidence by not only treating them with our services but also educating them about proper skin care at home. I also love to discuss how nutrition plays a great role in reaching optimal results.
After earning Laser Certifications as well as a Master Aesthetics Certification from NIMA
(National Institute of Medical Aesthetics). I decided to take my passion further and went on to work in the Dermatology field as well as the Luxury Spa field which I feel has given me a well-rounded education in this very competitive profession. Furthermore, I have had the opportunity to instruct professional aesthetic classes and also got to experience the management world for a couple years at a busy medical spa. My experience in all of these areas have given me the opportunity to apply my favorite treatments with clients, but has also made me realize the treatment room is where I'm most happy! My job is so rewarding and I’m so grateful to be able to do what I do! In my free time I love to hike, fish, paddleboard, dirt bike, and really anything outside. But, most of all, I enjoy spending time with my 3 kiddos!
